Before you start
Please read and accept these points. You only need to do this once on this device.
- Personal record, not official. What you enter is your own record. It is not an official immunization record, and printouts are not proof of vaccination.
- You are responsible for accuracy. Check dates and details against official records from your health care provider or state registry.
- Stored only on this device. MedRise does not receive, see or keep your records. If this device is lost, the browser data is cleared, or you forget your passcode, MedRise cannot recover them. Keep backups.
- Not medical advice. This tool does not tell you which vaccines anyone needs, whether anything is due or complete, or what a lab result means. Ask your health care provider.
- Adults only. You are 18 or older and entering information about yourself or family members you are authorized to keep records for.

Backups and exports are plain files saved to your device. Keep them somewhere private; anyone with the file can read it.

Your records are encrypted on this device with the passcode. If you forget it, the records cannot be recovered, so keep a backup.
Change history (0)
Every addition, edit and deletion is listed here with its time, so you can see what changed. It is stored with your records on this device and included in backups. Entries about deleted records stay here until you remove that person (which also removes their history) or use "Clear all my data".
