Do not enter patient information

Our professional and education tools are not designed to receive, store, or transmit protected health information (PHI) or any information that identifies a patient. Do not enter patients' names, dates of birth, record numbers, or other identifiers into those tools or into any form. We are not acting as a business associate under HIPAA for any user.

Personal record-keeping tools are different. Some tools, such as Family Vaccine Record, are made for you to keep your own and your family's information. Those tools store what you enter only on your device and never send it to us; see Consumer health data below.

What stays on your device

Most tools run entirely in your web browser. Anything you type into a tool is processed on your device. Where a tool saves your work between visits, it uses your browser's local storage, which stays on that device until you clear it. We cannot see it.

What we receive

• Early-access and contact form: the name, email, role, plan interest, and optional message you submit. Submissions are stored in our account with our hosting provider, Cloudflare, and are passed to our Google Workspace account so a copy reaches [email protected] and you receive an automatic confirmation. Our team replies from [email protected]. We use them to reply to you and to email you about launches you asked about.
• Server logs: our host may record standard technical data such as IP address, browser type, and pages requested, for security and reliability.

We do not use advertising trackers, sell personal information, or share it for cross-context advertising.

Consumer health data

Some MedRise tools, such as Family Vaccine Record, let you keep health information about yourself and your family. This section is our consumer health data privacy notice, including for Washington's My Health My Data Act and California's Confidentiality of Medical Information Act.

• What we collect through these tools: nothing. Information you enter in a tool is processed and stored only in your browser on your device. It is not sent to MedRise, our hosting provider, or anyone else, and we have no way to see it.
• Sources and purposes: you are the only source, and the only purpose is to show the information back to you, print it, and export it when you choose.
• Sharing and selling: we do not collect, share, or sell consumer health data, and we do not use it for advertising. Our tools contain no advertising or analytics trackers.
• Your acknowledgment: when you accept a tool's first-use acknowledgment, the tool saves only the acknowledgment version and the time you accepted it, on your device. It contains no health information and is not sent to us.
• Files you create: printouts, spreadsheets, calendar files and backups are saved by you to your own device. Anyone with the file can read it, so keep it private.
• Deleting it: use the tool's "Clear all my data" control, or clear this site's data in your browser. Because we hold no copy, there is nothing for us to delete, but you can email [email protected] with any question.
• Early-access sign-ups: the early-access form asks for your name, email, role, the tool and plan you are interested in, and an optional message. Please do not include health information about yourself, your family, or patients in the message. Signing up does not give us any of the records you keep in a tool.
• Your rights: you can ask us to confirm what personal information we hold about you (for sign-ups: the form fields above), to give you a copy, to delete it, or to withdraw consent, by emailing [email protected]. We respond within 45 days. If we decline a request, you can appeal by replying to our answer with "appeal" and we will review it and respond in writing within 45 days; if you are still not satisfied, you may contact your state attorney general.

If a future feature would send health information off your device (for example, sync between devices), we will describe it and ask for your separate, explicit consent before it is turned on.

Your choices

You can unsubscribe from launch emails at any time by replying "unsubscribe," and you can ask us to access or delete your form submission by emailing [email protected].

Children

MedRise is intended for adults and professional or student users. It is not directed to children under 13, and children should not use it themselves. In personal record-keeping tools, a parent or guardian may enter information about their own children; that information stays on the parent's device and is not collected by us.

Changes

If we change this notice, we will update the date above. Material changes that affect tools which send data off your device will be described on that tool's page before you use it.
