Terms of use and medical disclaimer

Not medical advice
MedRise tools provide general educational information and administrative or workflow support. They do not diagnose, treat, or prevent any condition, do not recommend any specific treatment for any individual patient, and are not a substitute for the independent judgment of a qualified healthcare professional. Always verify outputs against current primary sources, institutional policy, and product labeling before acting on them. In an emergency, call your local emergency number.
No patient information
Do not enter protected health information or patient identifiers into any professional or education tool or into any form. Those tools are not designed or offered for that use.
Personal record-keeping tools
Some tools, such as Family Vaccine Record, let you keep your own and your family's health information. Information you enter is stored only on your device, under your control; we do not receive it and cannot recover it if it is lost or if you forget a passcode. These tools keep records only: they do not recommend vaccines or treatments, decide whether anything is complete or due, or interpret lab results, and their printouts are personal records, not official records. You are responsible for the accuracy of what you enter and for keeping backups. Do not use a printout or export as, or alter it to look like, an official or government record.
Before first use, these tools ask you to confirm that you have read and agree to the following, and the tool remembers that confirmation on your device:
- The record is a personal record, not an official immunization or medical record, and printouts are not proof of vaccination.
- You are responsible for the accuracy of what you enter and will check it against official records.
- Your information is stored only on your device. ViaAI LLC does not receive it and cannot recover it if your device is lost, your browser data is cleared, or you forget your passcode; keeping backups is your responsibility.
- The tool does not provide medical advice and does not determine which vaccines anyone needs, whether anything is due or complete, or what a lab result means.
- You are 18 or older and enter information only about yourself or family members whose records you are authorized to keep.
To the fullest extent permitted by law, ViaAI LLC is not responsible for loss of information stored on your device or for decisions made using information you entered. Nothing in these terms limits any right you have under applicable privacy or consumer protection law.
Accuracy and availability
We work to keep tools accurate and tested, but they are provided "as is" and "as available," without warranties of any kind, express or implied, including fitness for a particular purpose. Content may contain errors or become outdated. We may change or discontinue a tool at any time.
Limitation of liability
To the fullest extent permitted by law, ViaAI LLC is not liable for any indirect, incidental, or consequential damages arising from use of the tools. Our total liability for any claim relating to a paid plan is limited to the amount you paid for that plan in the three months before the claim.
Paid plans
Where a paid plan is offered, its price, billing interval, and what it includes are shown on the tool's page before purchase. Payments are processed by a third-party payment provider. You may cancel a subscription at any time; access continues to the end of the paid period.
Acceptable use
Do not misuse the tools, attempt to disrupt the service, or present tool outputs as professional advice to others.
