QI Family of Measures Planner
Plan the measures for your QI project.
No patient information. A measurement plan describes how you will count things, not who the patients are. Never enter patient names, record numbers, dates of birth, phone numbers or other identifiers. Write roles (such as "clinic nurse") rather than staff names. Your plan stays in this browser and is never sent to MedRise.
Not saving. This browser is refusing to store your changes (storage full or blocked). Download a backup in step 6 before you close this page.
Saved data could not be read. It may have been damaged, so the tool started with a fresh project and set the unreadable data aside in this browser (Clear my data removes it). If you have a backup file, restore it in step 6.
1. Project
A clear aim has a number and a date. The plan check looks for both.
2. Measures
Outcome measures show the result for patients or the system. Process measures show whether the steps you changed are happening. Balancing measures watch for new problems your change could cause elsewhere. Fields marked * are needed for a complete operational definition.
3. Plan check
Checks your plan against common measurement-plan guidance: a small family of outcome, process and balancing measures, complete operational definitions, enough data points for a run chart, and no identifiers. It checks completeness, not whether these are the right measures for your project.
Shaded band: the typical range (1–2 outcome, 3–5 process, 1–2 balancing measures).
4. Changes since sign-off
When your mentor, faculty advisor or QI committee approves the plan, record the sign-off here. Every later change to the aim or the measures is listed below, so you can show exactly what changed.

5. Plan summary
6. Downloads and data
Measurement plan
One row per measure with its full operational definition, for your project file or a spreadsheet.
Data collection sheet
A blank sheet with one row per data collection period, from your start and end dates and the measure's frequency. Percentages and rates calculate themselves when you fill in the numerator and denominator. Record totals for each period only, never patient-level rows. Then chart the values in the QI Run Chart Builder.
Your data
Everything is stored only in this browser. A backup holds all your projects, including sign-offs, and can be restored on another device.
Education and workflow support only. QI Family of Measures Planner helps you write down and check a measurement plan. It does not choose measures for you, does not judge whether a project is quality improvement or research (ask your organization's IRB or QI office), does not analyze patient data and does not replace your organization's QI process or professional judgment. Not for patient information. See the terms and disclaimer and privacy notice.
