Hazard Vulnerability Analysis Tool
Score your hazards. Rank your risks. Plan the fixes.
No patient information. This tool is for facility-level planning. Never enter patient names, record numbers, room numbers or any detail about an individual patient. Your data stays in this browser.
Your assessment
Which hazards apply to you?
A starter list of common healthcare hazards. Untick any that cannot affect your site and note why (the reason prints in the report), or add your own. Use "Mark not applicable" on the Score tab instead if you want the hazard scored as 0%.
How to score (0–3 scales)
Probability
How likely is this to happen here? 0 not applicable · 1 low · 2 moderate · 3 high. Use history, local hazard data and vendor or utility records.
Impact (human, property, business)
If it happened, how bad? 0 none · 1 low · 2 moderate · 3 high. Human = injury or death of patients, staff, visitors. Property = damage, repair, replacement. Business = services interrupted, staff unable to work, supplies, revenue, reputation.
Mitigation (preparedness, internal, external)
How ready are you? Scored in reverse, so weaker readiness raises risk: 1 strong · 2 moderate · 3 weak or none. Preparedness = plans, training, drills. Internal = your own staff, supplies, backup systems. External = community, vendors, mutual aid.
Relative risk = (probability ÷ 3) × (sum of the six impact and mitigation scores ÷ 18) × 100%. Probability 0 means not applicable and the risk is 0%.
No hazards scored yet. Score at least one hazard on the Score tab, or load the fictional example under Data.

Add a mitigation action
Turn your highest risks into owned, dated actions: a drill, a plan update, a backup supply, a vendor agreement.

Report and export
The printed report has your assessment details, the ranked hazards with every score, the risk matrix and the action plan. In the print dialog choose "Save as PDF" to keep a file for your emergency management binder.
Backup, example and reset
Restoring a backup or loading the example replaces what is saved in this browser, so download a backup first if you want to keep it. Everything is stored only in this browser's local storage.
